Complete Official TranscriptFull transcript covering your entire programme.

O'Level CertificateWAEC, NECO, NABTEB, GCE, SSCE, IGCE, GCSE, etc. — 5 credits including English & Maths (max 2 sittings).

Passport Data PageUpload the page showing your photo and personal details. If you used more than one passport during your study, upload all of them.

Entry Visa / Study Permit / Residence PermitUpload all available documents showing your legal stay in your country of study.

First Departure Stamp (from Nigeria)Stamp for the first time you left Nigeria to begin your studies.

First Return Stamp (after study)Stamp for your first return to Nigeria after completing your programme. If you returned through E-Gates and have no stamp, explain this during physical verification.

Profession-Specific Requirements

Medical Doctors / OptometristsEvidence of completed housemanship + registration with MDCN or Optometrists Association.

PharmacistsEvidence of completed internship + registration with the Pharmacists Council of Nigeria.

Nurses / MidwivesValid nursing licence + registration with the Nursing and Midwifery Council of Nigeria (NMCN).

Medical Laboratory ScientistsValid practising licence + registration with the Medical Laboratory Science Council of Nigeria (MLSCN).

Missing Documents?
Loss of International Passport
NYSC requires:
- Police report
- Immigration report (from the country where it was lost)
- Sworn affidavit
- Evidence of stay in your country of study
- New international passport (upload the data page)
This helps NYSC confirm your travel history and identity.
Loss of Academic Credentials
If you lost your O'Level, degree, or HND certificate:
- Police report
- Sworn affidavit
- Confirmation of result:
- O'Level → from the exam body (WAEC/NECO/etc.)
- Degree/HND → from your institution
NYSC accepts these confirmations during registration.
