Documents Checklist

Gather these early — some take time to obtain.

Core Documents
  • Degree or HND CertificateOr a Statement of Result if your certificate isn't ready.
  • O'Level CertificateWAEC, NECO, NABTEB, GCE, etc. — 5 credits including English & Maths (max 2 sittings).
  • School ID CardOriginal school-issued ID.
  • JAMB Admission Letter
  • Birth Certificate or Age Declaration
  • Passport PhotographsAt least 10 copies, white background.
  • NERD Clearance SlipMore information on how to obtain this is provided below.
  • Marriage Certificate (Optional)For married women requesting concessional posting.
Profession-Specific (If Applicable)
  • Medical Doctors / OptometristsHousemanship proof + MDCN/Optometrists registration.
  • PharmacistsInternship proof + Pharmacists Council registration.
  • Nurses / MidwivesValid nursing licence + NMCN registration.
  • Medical Laboratory ScientistsMLSCN licence + registration.
How to Get NERD Clearance Slip (Graduates from Nigerian Institutions)

Prepare the following before starting the process:

  • Final Year Project or Thesis (Soft Copy)
  • Degree / HND Certificate or Statement of Result
  • National Identification Number (NIN) or NIN Slip
  • Passport Photograph
  • Functional Email Address

Applicants are advised to use approved agents or accredited centres for enrolment support. Find an accredited centre here: Accredited centers.

After getting the slip, download and save the PDF of the NERD Clearance Slip for NYSC registration.

Missing Documents?

Loss of International Passport

NYSC requires:

  • Police report
  • Immigration report (from the country where it was lost)
  • Sworn affidavit
  • Evidence of stay in your country of study
  • New international passport (upload the data page)

This helps NYSC confirm your travel history and identity.

Loss of Academic Credentials

If you lost your O'Level, degree, or HND certificate:

  • Police report
  • Sworn affidavit
  • Confirmation of result:
    • O'Level → from the exam body (WAEC/NECO/etc.)
    • Degree/HND → from your institution

NYSC accepts these confirmations during registration.
